Claire was a 36-year-old woman with a long history of substance misuse and type II diabetes. For many months prior to admission, she had not been registered with a GP or taken any of her prescribed medicines and had never engaged in any substance misuse services. 

After being admitted to hospital, Claire was identified as a resident of a homeless hostel and referred to the Inclusion Health Team who were able to assist in preventing her hostel from closing her bed space due to non-attendance. The team were also able to support her in registering with a GP and accessing drug services in her area. 

Following discharge, the team arranged for the Groundswell peer advocacy service to support Claire to attend her follow-up appointments, which were required to maintain her treatment at her GP, substance misuse service and hospital outpatient clinics.
