Pathway is pleased to share the newly published Inclusion Health & Safeguarding Self‑Assessment Tool.
People from inclusion health populations experience some of the poorest health outcomes and highest rates of premature and preventable mortality. Effective multi‑agency safeguarding is essential to improving these outcomes, and a key driver of success in delivering the commitments outlined in the NHS 10‑Year Health Plan’s and the National Ending Homelessness Plan.
Developed by Pathway Fellow Gill Taylor, in partnership with Groundswell’s London Participation Network and a range of cross-sector partners, the tool is designed to help NHS organisations, ICBs and their partners to strengthen local safeguarding systems for people experiencing homelessness and multiple disadvantage.
Intended to be multi-functional, the tool is flexible for use by single agencies, neighbourhood collaborations or system partnerships. It provides a structured way to review safeguarding practice, leadership, governance, and system readiness to respond effectively to abuse and neglect experienced by inclusion health groups and can be completed in part of full according to locally identified need. During piloting, local systems used the tool after serious incidents or Safeguarding Adult Reviews, for quality improvement, and to support implementation of policy commitments and new services.
Importantly, this tool forms part of a suite of inclusion health resources being published by OHID on NHS Futures, including the Inclusion Health Data & Intelligence Dashboard, the Inclusion Health Self‑Assessment Tool, the Inclusion Health Services Mapping Tool, and the HEAT Tool. Together, they aim to help local systems better understand needs, strengthen partnerships, identify gaps, and drive equitable, person‑centred care.
The Inclusion Health & Safeguarding Self‑Assessment Tool will be published soon on the Inclusion Health Learning Space on NHS Futures.
